Required Forms

Listed here are the forms needed to be filled out. We ask that both forms, along with payment (money order, check, or Venmo), be completed and sent to the PO Box (this is preferred), or sent to the Pride float email listed above.

Once forms are filled out and money is sent, all information including confirmations and information about the camp address will be sent to the individual listed.
